Farewell Letter from Dr. Christina

Dear Patient,

I write this letter with mixed emotions, to inform you that I will be closing my Naturopathic practice at Kingston Integrated Healthcare (KIHC) at the end of November. I will be moving to the Hamilton/Brantford area to work at an Aboriginal Health Centre on an exciting provincial health promotion initiative called the Healthy Kids Community Challenge. This decision was not an easy one to make, as I love Naturopathic Medicine and all the wonderful connections I’ve made with patients over the years. I have enjoyed working with you and helping you on your health journey and I have enjoyed working with the team members at KIHC, however this new opportunity would allow me to continue my personal growth.

My last official day at KIHC is November 27th 2015, however I will be back for one final day on Saturday, December 12th for appointments. Anyone I have seen can choose to continue care with Dr. Sonya Nobbe, ND or Dr. Holly Whiteknight ND at KIHC. As well, there will be a new Naturopathic Doctor joining the KIHC team in February to potentially take on some of your patient files. Active patients (anyone I have seen since January 1st, 2015) may not require a full initial appointment from the new practitioner at KIHC, but this is at the discretion of the practitioner and may depend on the complexity of the case. I can provide your new KIHC Naturopathic Doctor with a brief letter on your behalf, outlining main concerns, treatment plans and a short summary. If you have been purchasing supplements from KIHC, you can continue to do so, however depending on the nature of the supplement, you may need to consult a Naturopathic Doctor for continued renewal.

For those of you choosing to see another Naturopathic Doctor within the Kingston community, or for patients who are no longer considered active, I can still provide assistance on your transition to another Naturopathic Doctor. Case summaries, file copies and referral letters can be provided for a fee. In these cases, a new patient appointment will most likely be required with your new Naturopathic Doctor.

If you require information from your file (such as lab reports), I can provide you with copies upon request free of charge for up to 10 pages. However, if you require a copy of the entire file for either yourself or another practitioner this can be arranged for a fee.

All requests for referral information, case summaries and file copies should be made no later than February 29th, 2016. After this time, although they will still be available, it may be difficult for me to provide these in a timely manner. After March 1st, 2016, all requests for file copies and case summaries will be subjected to a fee. If you would like to request a file copy and/or case summary, for another Naturopathic Doctor, please complete the attached form and return it to KIHC.
