Before you enter our facility, please take note of these clinic policies:
Scent-free policy: Some clients experience severe health symptoms when exposed to fragrances such as fabric softener, hair spray, and hand moisturizers. Please refrain from using scents while visiting the clinic. Appointments may be re-scheduled at the time of arrival if this policy is not respected. Thank you for understanding.
Cancellation policy: Please provide at least 24 hours business-day cancellation notice so that we may offer your appointment space to someone else. Late cancellations are subject to a 50% late cancellation fee.
INTAKE FORMS
Our new patient form is easily completed online through our secure “Jane” software program. This form will be emailed directly to you when you schedule your first appointment. Please complete it well in advance of your scheduled appointment time.
If you do not have an email address or if you’re unable to complete online forms, please download and print this pdf version instead.
New patient appointments are comprehensive and last about 60 to 90 minutes. Please come prepared with copies of your recent medical tests and reports if you have access to them.
