Before you enter our facility, please take note of these clinic policies:
Scent-free policy: Some clients experience severe health symptoms when exposed to fragrances such as fabric softener, hair spray, and hand moisturizers. Please refrain from using scents while visiting the clinic. Appointments may be re-scheduled at the time of arrival if this policy is not respected. Thank you for understanding.
Cancellation policy: Please provide at least 24 hours business-day cancellation notice so that we may offer your appointment space to someone else. Late cancellations are subject to a 50% late cancellation fee.
New patient forms are easily completed online through our secure “Jane” software program. These forms will be emailed directly to you when you schedule your first appointment. Please complete these well in advance of your scheduled appointment time.
If you do not have an email address or if you’re unable to complete online forms, please download and print these new patient forms (below), instead:
New patient appointments are comprehensive and last about 60 to 90 minutes. Please come prepared with copies of your recent medical tests and reports if you have access to them.
Initial assessments are 1.5 to 2 hours. Please wear light-weight comfortable clothing (or bring these with you to change into), since heavy clothing and thick seams interfere with a proper hands-on assessment.
Please complete this form before your initial acupuncture session with our Registered Acupuncturist, Jianmin (Jamie) Xu. These sessions last up to 90 minutes. Please wear loose, comfortable clothing.
Initial consultations with our Speech Language Pathologist generally last 60 to 90 minutes.
These 50 minute sessions with our Registered Social Worker are available by secure video or in-person, upstairs at KIHC.
These sessions generally last up to 1 hour. Please refrain from alcohol and non-prescribed drug use for at least 24 hours before your appointment.
Video consultations are available for new and current patients across Ontario. However, telemedicine is not ideal for everyone and patients should be willing to attend in-person appointments at least once every 6 to 12 months, or as required for safe and effective care. Telemedicine appointments are billed at the same rate as in-person appointments.
